There are a number of forms that we require in order to facilitate the Internship process: All forms are on-line and the results will be sent directly to the field education email.
Initial Internship Forms
This is a great guide to help you determine when all of the documents are due and need to be submitted.
Initial Planning for Internship
This form is completed by the intern and mentor during the Internship Seminar.
This form is signed by all intern candidates and gives Ambrose University permission to talk to a church or not-for-profit organization about an internship placement for you.

Police Check see Police check information sheet
This document includes important information about how to apply for a police check (with vulnerable sector search) if you do not have a police check that is less than 12 months old. [Note: we do not require the original copy of your police check and you are allowed to submit a scanned copy of your police check]

Registering for Internship Course
Once student’s internships have been confirmed and the Internship Seminar completed, the field education department will contact the registrar on your behalf advising the length of your internship. You will registered in the appropriate semesters, and then will receive an email from the registrar confirming registration.
Undergraduate Students
- FE 300a/b – Church Ministries/ Children and Family Ministry/ Youth Ministry/ Worship Ministry Internship (6 credits)
- FE 365a/b – Intercultural Ministries Internship (6 credits)
- FE 400 – Transition to Vocational Ministry (3 credits) > offered during module week in winter semester
Seminary Students
- PT 712/MI 712 – Ministry Internship (3 credits)

Use this training model document to help create your internship learning contract. Training models include: not-for-profit, pastoral, family/children, and youth.
Final Internship Debrief:
Once all of your documents have been submitted you will need to schedule a final internship debrief interview with Rev. Dr. Ric.Strangway@ambrose.edu in order to complete the Internship process. Please be mindful that all of your documents are required to be submitted before you can complete this interview.
Note: a final grade for the internship course will be submitted to the Registrar’s Office after the debriefing meeting
